Oral Health Exams and Assessments

You will be asked to fill out relevant personal, medical and dental information that will be used to assess your general health condition to ensure that you are a safe candidate for dental cleanings and to determine if medical consultations with your Doctor or Specialist are required prior to treating you. All information collected will be used to formulate a dental hygiene diagnosis so that we can devise an appropriate client specific and tailored treatment plan based on your needs. The information you provide will reveal if there are any health issues that may be contributing factors to your oral health and it will help us determine your risk for cavities, periodontal/gum disease and oral disease.
